Updated (14/09/2018): It's now possible to duplicate multiple pages at once, instead of just one at a time.
Using this script you can duplicate a page in a PDF file one, or more, times, while preserving the form fields on it in tact. There's even an option to automatically rename the form fields so that they could have unique values.
This is a fast and easy way of creating multiple copies of a one-page form, or to set up a file for n-Up printing.
When you run the script you're presented with this dialog window, where you can select which page(s) to duplicate, how many copies to create and whether the fields on those pages should be renamed (so each copy is unique) or not:
When you press OK the tool will immediately duplicate the selected pages and place the copies at the end of the current file.

You get it automatically via email once you purchase it, using the Buy Now button at the top of the page.

When you multiply the pages, does it place them at the end of the file or insert them next to the page it is duplicating?
The new pages are inserted at the end of the document. You can later move them manually to another location if you wish.
I have a two-page PDF form that consists of:
1) a primary page
2) a secondary page
I want to automate the PDF so that a user with Acrobat Reader can add multiple copies of the secondary page, with the copies having unique field names.
The user would be a sales rep at my company who’s preparing a client contract. The sales rep should be able to duplicate Schedule A but the client should not.
QUESTION 1 → Obviously one would need Acrobat Pro to add your duplication script to the “master” PDF but will the script run for a user of Acrobat Reader?
QUESTION 2 → Can the “duplicate a page” functionality be password protected so that only users who know the password (e.g. our sales reps) can use it?

hi, i will be duplicating two form page that I have pre populated with dropdown lists, will these be duplicated with unique numbers and retain the list properties. thanks

Yes, the fields will have unique names, but the rest will be exactly the same.
